Case Management

Our community-based case management is available for FREE to survivors of all ages and incomes in the Brain Injury Solutions (BIS) service area.

​

Direct case management services are designed to meet the unique needs of survivors of brain injury by linking survivors with medical services, individual and family counseling, cognitive re-training, recreational activities, and positive behavioral training. BIS also helps survivors and their families advocate for care that will help meet their needs. Case management services are individualized and community-based. BIS provides case management services to pediatric consumers (under 18) including helping families learn about their child's injury and provide advocacy support and educational supports such as with IEPs.

The goal of case management is to assist survivors in becoming more independent, achieving their goals in recovery, and reintegrating into their communities. This includes though not limited to support in the following areas:

 

  • brain injury education

  • housing solutions

  • finding recreational activities

  • transportation solutions

  • homemaking

  • coordination of school support services

  • problem solving

  • homeless shelter services

  • social interaction

  • finding a place to volunteer

  • medication management routine

  • preparing for a job

  • budgeting

  • mental health referral

  • finding and acquiring equipment

  • applying for benefit programs such as SSI, DARS, SNAP, etc.

  • finding or communicating with a healthcare provider

Serving all of Southwest Virginia and parts of Central and Southside Virginia including Alleghany, Bedford City, Bedford County, Bland, Botetourt, Bristol, Brunswick, Buchanan, Campbell, Carroll, Charlotte, Covington, Craig, Danville, Dickenson, Floyd, Franklin County, Galax, Giles, Grayson, Halifax, Henry, Lee, Lunenburg, Lynchburg, Martinsville, Mecklenburg, Montgomery, Norton, Patrick, Pittsylvania, Pulaski, Radford, Roanoke City, Roanoke County, Russell, Salem, Smyth, Tazewell, Washington County, Wise, and Wythe.

Eligibility

Services are provided to survivors of brain injury including people who have survived a car accident, fall, stroke, brain tumor, etc. Participants must reside in Brain Injury Solutions' service area, have documentation of a brain injury, and be willing to participate.
